Admissions and Forms

Registration continues for a few open spots in our 2012-2012 school year.  Please download and print out the Health Assessment Record (2 pages) at left, complete them, and mail them to us at the address provided below.

If you wish to speak with us regarding possible admission prior to completing the forms, please see our contact page to reach us.

Registration for the 2012 - 2013 commenced on January 1, 2012.  Please complete the Health Assessment form via the link at the left of this page, complete the form, and mail to Happy Time at:

Once we receive your completed Health Assessment record, we will send you a Happy Time Preschool Enrollment Form.

We must receive the last month's tuition payment (see Enrollment form for tuition amount) with your Enrollment forms, or the forms can not be processed.  Thank you!
